Project HOPE and Boston Scientific Celebrate Three Years of Transformative Health Impacts in Greater Houston

Project HOPE and Boston Scientific are proud to share the successful conclusion of their multi-year partnership, “Empowering Community Health Workers to Save Lives in Houston.” Since 2023, this initiative has fundamentally strengthened the healthcare safety net for underserved populations in the Greater Houston area, utilizing a community-centered approach for chronic disease prevention. Working in close collaboration with the Ibn Sina Foundation, the program improved the prevention and management of non-communicable diseases (NCDs), such as diabetes and hypertension, through their Avicenna Clínicas, which provide healthcare to un- and under-insured Texans.

The program was designed to address the “whole patient” through a comprehensive suite of support services that ensure continuity of care. To encourage patient participation, the program provided incentives, including free medications, rides to medical appointments, monitoring devices, and follow-up consultations to those who successfully hit their health targets, such as reducing A1C levels. Beyond physical treatment, the program emphasized holistic wellness by integrating mental health screenings and nutritional counseling, ensuring that patients felt supported beyond a simple clinical diagnosis. It also provided robust training for community health workers and volunteers to effectively navigate linguistic and financial barriers facing patients.

“The support from Boston Scientific has enabled Project HOPE to create a program that successfully and holistically supports chronic disease management for un- and under-insured people in the Greater Houston area who typically face significant barriers to accessing care,” said Anna Jackson, Domestic Program Manager for Project HOPE. “By investing in and building the capacity of local health workers, we have built a bridge to ensure that community members can access essential health services, regardless of their financial or medical situation.”

Over the course of the three years, the program helped to improve health outcomes for tens of thousands of people across the Greater Houston area:

  • Screening & Detection: Screened more than 21,300 individuals for chronic diseases, with over 4,800 diagnosed in 2025.
  • Health Outcomes: Helped to successfully reduce A1C levels by at least one full point, a critical marker for diabetes management, for 432 patients in 2025.
  • Health Worker Training: Cumulatively trained over 230 health workers and volunteers in specialized culturally competent NCD management and evidence-based clinical workflows.
  • Community Education: In three years, the program reached more than 90,000 community members with education and outreach initiatives that provided vital health literacy and resources.

After three years of dedicated programming, the Ibn Sina Foundation has integrated these evidence-based workflows and data tracking systems into its permanent operations, ensuring the program’s impacts have been institutionalized into the community.
